Community Service Form
Students in the class of 2009 and beyond must have 10 hours of community service before a high school diploma will be issued. These hours may be accumulated at any time in which the student is in high school. This can include the summer immediately before the student's freshman year. This form will be used to log and verify 10 clock hours of community service. Students should turn this form in to the Counseling Office when the 10-hour requirement is completed. (In other words, do not turn in one hour of service at a time.)

Transcript Release Form
This form will authorize the CHS Counseling Office and/or Principal's Office to provide all necessary transcripts for college and scholarship applications as requested by parents and students through the Counseling Office. Transcripts will not be sent without this authorization.

Attendance Appeal Form (instructions)

A high school student shall be allowed no more than eight (8) absences per semester in any one class (this includes partial absences). Any absence from a class (including partial absences) will count toward the eight (8) absence maximum and will be noted on attendance updates. On the ninth (9) absence in any one class, the student will not earn credit for that class.

If the student or parent feels that justifiable or extraordinary circumstances have contributed to not meeting the attendance expectation, the student and parent may appeal to the Attendance Review Committee at their assigned secondary school. The review by the Attendance Review Committee shall be held within a reasonable time following the end of the semester. Completed appeal forms need to be submitted to the CHS Main Office to Mrs. Viestenz. 

The following will not count toward the eight (8) absence limit with proper documentation: (a) hospitalization, (b) death in the family, (c) mandatory court date, (d) religious observance, (e) documented doctor’s appointment, (f) medically documented illness, (g) COVID related quarantines for close contact or positive cases, (h) verified school counseling or office appointment, (i) documented dental/orthodontist appointment, (j) college visits (2 allowed for seniors; 1 allowed for juniors), (k) first five days of ISS or OSS, (l) school related activities in which the student is a participant
